Travel Plans — Provide any travel plans that involve your child. Parents or legal guardians have to accomplish Form DS-11, U.S. Passport Application, to apply for a new passport for their child. One goes with your passport to the State Department; the other is payable to the City of Mill Creek for processing. Face masks are required during the time of your appointment. Temperature checks will be taken at the door. The reservation system shows all availability only two weeks out from today’s date; New appointments are entered into the system every Tuesday morning as staff schedules are set.

The notarized statement must have a photocopy of the identification document of both the parents or guardians. We do not accept American Express at this time. All of the information needed to process your application is listed on the application andtravel.state.gov, passport section. If you are missing anything at the time of processing, your appointment will be cancelled.
Step 1: Start Early
It should take approximately four to six weeks to receive your passport by home sliding mail. Expedited passports will usually take two to three weeks. As of February 2018, an adult renewal is $110. If you need you passport fast and wish to have it expedited, it will cost an extra $60.If you have to file a DS-11 rather than a DS-82, you will also have to pay a $35 acceptance fee. In most cases, you can use the standard DS-82 passport renewal form.
Building Renovation Passports
If you need to renew a U.S. passport, you can look online at the U.S. Department of State’s website to find the nearest post office facility that offers passport services. Once you find the location where you want to renew your passport, call to schedule an appointment. If you want to fill out your passport renewal form ahead of time, you can print a copy from the Department of State’s website. When it’s time for your appointment, bring your renewal form, your old passport, a new photo if you want one, and a renewal fee of $110.
When you’ve completed the application, print it out to take to the passport acceptance facility. Passport applications cannot be submitted online. If you will be traveling in more than eight weeks, you can follow the regular application process and apply in person at apassport acceptance facility. U.S. citizens residingin Canadamay submit Form DS-82 to one of the addresses listed on the form.

Renew A Passport From Outside The United States
Mailing delays are impacting passport processing. The Department of State’s processing times begin when applications are received at a passport agency or center and not when you mail them or apply at a local acceptance facility. Standard service may take between 8 and 11 weeks. This timeframe does not include delivery time.
